Port removal

A referral for port removal should be made out to a hospital, to the department of anaesthesiology and intensive care (department code 4260), and contain the following information:

purpose of the referral: removal of a foreign body from the skin and subcutaneous tissue with incision, 86.054,

justification: e.g. completion of chemotherapy.

If you do not have a referral, I can issue one. For that I need your PESEL number and information about what you were treated for.
